Who is Eligible?

Hospice of Chesterfield County offers home-based hospice care for residents of Chesterfield and Marlboro counties, or patients whose primary caregivers are residents of Chesterfield or Marlboro County.  Care is available to people of all ages who have a limited life expectancy of six months or less (as determined by their physicians when the disease progresses as expected). Hours & Coverage

Hospice of Chesterfield County is located at 700 West Boulevard in Chesterfield, 114 E. McGregor Street, Pageland, and 160 Second Street in Cheraw.  Chesterfield office hours are Monday through Friday: 9 AM to 5 PM. Cheraw office hours are Monday through Friday: 10 AM to 5 PM (Wednesday: 10 AM to 1 PM). Pageland office hours are Monday through Friday: 10 AM to 5 PM (Wednesday 10 AM to 1 PM) and Saturday: 10 AM to 2 PM. Coverage is available 24 hours a day, seven days a week through an emergency on-call service after our office is closed, weekends, and holidays. A qualified Hospice nurse is available to accept patient calls, referrals, and to arrange for patient services as needed. Guidelines for Referral

Hospice of Chesterfield County serves residents of Chesterfield and Marlboro counties. Family, friends, physicians or other healthcare agencies can make referrals. Admission to Hospice is based upon the patient’s needs and the recommendation of the patient’s physician. Patients appropriate for Hospice care should meet the following criteria: have a terminal illness, seeking care that is based on comfort rather than cure (also known as palliative care), have a responsible caregiver or agree to develop an alternative plan of care consistent with the patient's safety and needs, and agree to accept Hospice services. On admission, our referral liaison will visit the patient and family to discuss Hospice services, assess needs and initiate a plan of care.

Financial Information

Hospice of Chesterfield County admits patients to the hospice program based on need and regardless of their ability or inability to pay. Medicare, Medicaid, and most private insurance companies cover hospice services. The VA has a Hospice benefit available for their patients. Services prescribed by a VA physician require authorization from the VA Community Health Nurse Coordinator, who will contact Hospice with the appropriate information. Charitable donations allow us to provide Hospice services to patients without a payment source.
